All new, incoming students who will be residing on campus are required to log into the Lynn Student Health Portal and complete and submit necessary paperwork to be eligible to check-in to the residence hall.
The portal is not through Workday.
By following the step by step instructions below, you will complete and submit the following:
- Health History and Authorization Form 11-2024- to be completed by student.
- If under 18**, the form must be completed as described below, AND the parent must upload a copy of their driver’s license for proof of electronic signature
- Proof of a Physical Examination 11-2024– proof of a general physical exam, dated within the last 12 months of move-in date
- Immunization Records 11-2024–
- Proof of TWO MMR (Mumps, Measles, Rubella) vaccines OR proof of immunity via a blood test (“titers”).
- Proof of a Meningococcal ACWY Vaccine dated within FIVE years of move-in date.
Proof of Hepatitis B vaccine, or signed waiver – see instructions listed under E.
