- Proof of a general physical exam, dated within the last 12 months of move-in date
- Immunization Records 11-2024
- Proof of TWO MMR (Mumps, Measles, Rubella) vaccines OR proof of immunity via a blood test (“titers”).
- Proof of a Meningococcal ACWY Vaccine dated within FIVE years of move-in date.
- Proof of Hepatitis B vaccine, or signed waiver – see instructions listed below under 5.
