After submission, the Office of Academic Affairs will process the case via Lynn University’s case management system (Maxient).
The Office of Academic Affairs will notify case findings and resolution to students, faculty, and the registrar office via email letters. Definitions and procedures for Academic Dishonesty incidents are described in the academic catalog under section Academic Honesty Policy.

  1. Once you have accessed the form, Under Faculty Information, fill in the form with your name, email address, date and time of incident.

         



    2. Under Involved parties, please enter information about the student(s) involved in the incident. Enter Name and ID number. You can add more than one student by clicking the “Add another” button.

         


   3. Please complete all questions related to your course: Course number (e.g. ABC 100), name, a description of the incident and your recommended sanction(s)

       


Note: Faculty must submit supporting documentation for the incident (Turnitin reports, scanned exams, papers, etc). Multiple documents can be uploaded. 

  4. To delete an uploaded document, click the trash con next to the document. Documents must be 1GB maximum size, and we recommend providing PDF versions of the documents. Check “Email me a copy of this report” if you want a copy of the report emailed to you. Click submit the report to complete the form.
