  4. You can provide more than one document. To delete an uploaded document, click the garbage can icon trash con next to the document. Documents must be 1GB maximum size, and we recommend providing PDF versions of the documents. Check “Email me a copy of this report” if you want a copy of the report emailed to you. Click submit the report to complete the form.

After submission, the Office of Academic Affairs will process the case via Lynn University’s case management system (Maxient). The Office of Academic Affairs will notify case findings and resolution to students, faculty, and the registrar office via email letters.
